Investigation Summary
Employee #1 and coworkers were digging a 6 ft wide by 8 ft deep by 15 ft long trench from east to west to install a drain line for a newly constructed residence. The trench was straight-walled and adjacent to the residence. The soil was a layered rock formation, with layers at a downward angle from north to south about 45 degrees from horizontal. Employee #1 entered the trench to check if the downward slope was adequate for proper drainage. He was in the trench for only a short time when a large portion of the north wall collapsed. Employee #1 was struck in the legs and chest, and was partially buried. He suffered severe crushing injuries and blunt trauma, and died of compression asphyxia. The causal factors include failure to shore or slope the trench, failure to provide adequate employee training to recognize and avoid job hazards, and failure to recognize dangerous soil conditions.
