Investigation Summary
At approximately 8:15 a.m. on July 17, 1990, Employee #1 was greasing the end of a section of pipe in a trench that measured approximately 6 ft 6 in. deep by 6 ft 6 in. wide by 20 ft long. As he was preparing to lay the greased section of pipe, the north wall of the trench caved in, knocking him into the south wall and completely covering him. Employee #1 was pronounced dead at the scene.
