Investigation Summary

Investigation Nr: 170579130
Event: 10/11/1995
Employee killed in fall in elevator shaft

Employee #1 and a coworker were standing directly in front of elevator #11 on the 30th floor of the Marriott Marquis. They were waiting for two other coworkers to drop a line so that they could connect it to the hoist line and run it back up to the motor room. A 6 in. by 6 in. by 6 ft yellow southern pine column used to restrain the counterweight collapsed while under compression, and the counterweight dropped 16 ft. The cable and Employee #1 were pulled into the shaft of the elevator. Employee #1 fell 8 ft and was held suspended in the air by the cables in which his feet were entangled. He was killed when he fell as firemen were trying to rescue him.
