EMPLOYEE #1 WAS ATTEMPTING TO LOAD A FORKLIFT ONTO A FLATBED TRAILER. THE FORKLIFT FELL OFF THE TRAILER AND STRUCK EMPLOYEE #1. HE ATTEMPTED TO AVOID THE FORKLIFT, BUT INSTEAD JUMPED INTO ITS PATH. HE WAS KILLED.
