Investigation Summary
Employee #1 was working on a 7 foot 9 inch high work platform. He approached the edge of the platform to pick up some 2 inch by 4 inch lumber, and, according to an eyewitness, appeared to just roll off the edge and fall face down onto the concrete floor. Employee #1 was rushed to the hospital where a pre-existing blood clot was removed from his brain. He sustained no trauma injuries from the fall. It appeared that Employee #1 had suffered a stroke. The periphery of the work platform was not guarded by standard guardrails, which would have prevented Employee #1's fall.
