Investigation Summary
On January 13, 1995, around 7:00 p.m., Employee #1 was cleaning adhesive buildup off a conveyor belt of a corrugator machine with a long-handled scraper. A coworker was the machine operator. Both employees wore headsets to communicate. A coworker gave a verbal warning that he was going to restart the belt, which was approximately 88 inches across. The coworker stood on a 2 in. by 12 ft by 10 ft long board and gave the affirmation that it was okay to restart. A 4 second alarm sounded and then the belt started; a paper jam occurred. The coworker stopped the belt immediately to correct the problem. The coworker gave the warning again to restart; Employee #1 said it was okay. When the machine was restarted, a cry was heard to stop the machine. Employee #1's head had been caught between the belt and roller. He died.
