Investigation Summary
At approximately 8:30 a.m. on August 17, 1994, Employee #1 was test firing DA-38 double barrel derringers manufactured by the American Derringer Corp. On discovering a derringer with particularly good qualities, Employee #1 summoned Employee #2, the engineer, to the test firing range. Employee #1 handed the derringer to Employee #2 who assumed the firearm was unloaded and tested the trigger pull. The derringer, with both barrels loaded, discharged, firing one round that struck Employee #2 in the left index finger, then passed through and struck Employee #1 in the head. Employee #1 was hospitalized and died later that day. Employee #2 was hospitalized for reconstructive surgery on his finger.
