Investigation Summary
Employee #1 was lying on the floor working under a small shop-made trailer that was supported by three jack stands. Another employee, who helped him install the jack stands, left for about 10 minutes. When he returned, he found Employee #1 dead. Somehow, Employee #1 had knocked over the two rear jack stands and the trailer had fallen with the rear axle across his neck.
