Employee injured when pinned by conveyor

At approximately 11:45 a.m. on February 19, 1996, Employee #1 was assigned to inspect parts on the #4 packer conveyor in a feed mill building to ensure that a similar conveyor could be constructed for the #2 packer. A coworker was operating the #4 packer conveyor in normal production during the time Employee #1 initiated the inspection. The coworker had the conveyor raised 28 in. off the floor. Employee #1 crawled under the conveyor and instructed the coworker not to lower it. At 12:00 noon, the coworker went for lunch. At 12:22 he returned and found Employee #1 caught under the lowered #4 packer conveyor. Witnesses account for the removal of the gear box and the disassembly of the chain drive used for raising and lowering the conveyors. Employee #1 was given aid, taken by ambulance to the Mission Hill Hospital in Shawnee, and later transferred to the Presbyterian Hospital in Oklahoma City in critical condition.
