Employees killed when struck by lifting beam

A construction contractor was installing a mechanical power press in a manufacturing facility with the help of the plant's employees. A four point lift system with a main lifting beam was used to place the press in a pit that had been prepared earlier. During a final attempt to lift the press to remove debris, the main lifting beam swung down, struck, and killed Employees #1 and #2. Employee #1 worked for the contractor and Employee #2 was an employee of the facility. The workers did not ensure that the four lifting units of the four point lift were plumb and level at all times during the lifting and lowering of the load.
