Employee killed when buried by trench cave-in

On December 9, 1992, Employee #1 was in an approximately 7 foot deep trench installing a sewer lateral line at a new residential construction site. He was using a shovel to clean out loose dirt when one wall caved in and completely buried him; he was killed.
