Investigation Summary
On December 4, 1991, Employee #1, a pumpman for T. L. James, was assigned to watch a 1,765 lb Gorman-Rupp centrifugal pump that was placed on a 36 in. I-beam used as a whaler and lodged between two 6 in. channel irons that were welded to the sheet piling and the I-beam. The pump was in a 21 ft deep by 60 ft wide by 100 ft long excavation. This excavation was adjacent to a pumping station and filled with water. Employee #1 was at ground level at 6:30 a.m. and was instructed to observe the pump and the level of the water. He was left alone. At 9:30 a.m. a coworker walked by this area and noticed that the pump had fallen to the floor of the excavation. When employees went into the excavation to right the pump, Employee #1 was found underneath. The pump had dislodged from its placement and toppled over onto him. The pumpman performs no operation that requires him to be in front of the pump. The wind chill factor that day was 29 degrees F. No one witnessed the event. It is assumed that Employee #1 went into the excavation to escape from the cold and stood close to the pump's muffler, directly in front of the running and vibrating pump, in order to warm his hands. The muffler is positioned almost in the center of the pump. When the pump was removed from Employee #1's body, a muffler burn was found on his chest.
