Investigation Summary
On October 26, 1992, Employee #1 was attempting to clear a jam between gates #3 and #4 on a rotary log trimmer. He hooked up safety chains to the gates and tried to unjam the log. Believing that he had been successful, he unhooked the safety chains. As he started to walk out, however, he found that the log was still jammed. He then went back to the jam and, without hooking the safety chains back up, he got in between the jammed log and the open gates. The power went off and the gates closed, crushing him in between the log and gates and killing him.
