Employee dies after fall from ladder

On March 25, 1995, at approximately 8:50 a.m., Employee #1 was climbing an aluminum extension ladder to secure the platform for a ladder jack scaffold. Employee #1 apparently clenched his jaw, started to shake, and fell from the ladder. The ambulance was called and took him to a hospital. Employee #1 was pronounced dead on arrival; he may have suffered a stroke or a heart attack while on the ladder.
