Employee dies of chemical burns after falling into vat

Employee #1 was cleaning valves, when he went up onto a wash rack to clean the dome lids on a tank truck. When he reached the top of the rack, he walked across the grate and fell into an open vat that had its lid removed for refilling. Employee #1 sustained chemical burns from sodium metasilicate over 90 percent of his body. He died 6 days later.
