Investigation Summary
At approximately 5:37 p.m. on April 22, 1996, Employee #1 crawled on top of a suspended ceiling to close a water valve. The ceiling collapsed and he fell 14 ft, striking a display fixture before hitting the floor. He suffered severe head injuries and was taken to the hospital, where he underwent surgery the next day. Employee #1 died after the operation, as a result of his injuries.
