Investigation Summary
At approximately 1:00 p.m. on April 17, 1991, Employee #1 and his brother were staking out bar joists on the roof of the third floor mechanical area of the central pavilion. The employee was sitting astraddle on an I-beam 26 ft 7 3/8 in. above the second floor; his brother was sitting astraddle on an I-beam 34 ft 10 1/2 in. above the floor. A bar joist that was being moved into position slipped off the I-beams and began to fall. Employee #1 was pulled off the I-beam on which he was sitting. As he fell, he struck his head twice on I-beams and suffered severe head injuries and a fractured skull. He was killed.
