Investigation Summary
On September 22, 1991, Employee #1 was showing a prospective buyer a house. He used a pull-down ladder in the garage to access the roof to inspect the insulation. The employee was standing on the ladder's fifth rung, about 4 ft 7 in. from the concrete floor, when the ladder unit collapsed, causing the employee to fall to the floor. He landed headfirst on the floor and lost consciousness. The employee was taken by ambulance to the hospital, where he was diagnosed as comatose. Employee #1 underwent two surgeries within a week but died on May 29, 1991.
