Investigation Summary
At approximately 8:15 a.m. on August 29, 1991, Employee #1 was helping a forklift operator load 6 in. by 44 to 48 ft steel pipes onto a semi-trailer. The employee was standing on top of the load attempting to roll the steel pipe off the forklift fingers and into a secured position on the top layer of the load. The employees were loading the top layer when a steel pipe came off the fingers and went over the side of the trailer. Several other steel pipes followed, knocking Employee #1 off the trailer and then striking him on the head as he lay on the ground. He was killed.
