Investigation Summary
Between 7:05 and 7:10 a.m. on September 3, 1996, Employee #1 and two coworkers arrived at the fuel pumps outside the north side of the Highway Department garages. They intended to fuel a 1988 GMC dump truck and a roller paver that sat on a trailer attached to the truck. Employee #1 told his coworkers that he felt nauseous and ill. He set a soft drink cup on the steps below the truck's driver side door and moved out of sight. When the coworkers finished fueling the roller and the truck, one of them climbed into the truck and checked its mirrors. He saw the coworker who had helped him fuel the truck still on the trailer behind him, and nobody in front of him. At approximately 7:15 a.m. the driver began to inch the truck forward. He had moved approximately 8 ft when he felt as though he had rolled over something. He traveled another 6 ft, stopped, and got out of the truck. The driver found Employee #1 lying on the ground next to the front right tire, having sustained severe injuries to his arm and head. The driver went to the garage office to summon help. The Jackson County ambulance service treated Employee #1 at the site and he was then transported by STAT flight to the University of Louisville (KY) Hospital. Employee #1 died at 4:58 p.m. of multiple traumatic injuries.
