Investigation Summary
At about 4:25 p.m. on October 12, 1991, Employee #1 was helping a coworker remove a die holder from the bed of a press brake. While the employees slid the holder onto the forks of a fork truck, the die holder tipped over and caught Employee #1's thumb between the holder and the die bed. The employee went to the company nurse, then to a doctor. He sustained avulsion of a nail, a laceration, and a fractured bone in his right thumb. He returned to work the next day and worked with some restriction.
