Investigation Summary
At approximately 12:00 noon on September 1, 1995, Employees #1 through #5 were removing a section of pipe from a 2,000 psi high pressure gas line that was supposedly empty. Just before the pipe cutter opened the pipe, it exploded with 1,500 psi still in the line. The explosion threw Employees #1 and #2 in the air, killing them. Employees #3, #4, and #5 were injured. All five employees suffered concussions.
