Investigation Summary
Employee #1 was operating an onion dispenser on a production line where frozen TV dinners are manufactured. She was using a plastic handheld scoop to move pearl onions out of a box and into the dispenser. The bottom plate of the dispenser had six holes through which the onions dropped onto a dinner plate that was passing on a conveyor line below. The bottom plate revolves continuously and a shear point is created by a metal bar that holds the onions back so that they drop down into the holes on the plastic disc plate. The employee had her right hand in the dispenser breaking up some frozen onions that were stuck together when the tip of her index finger went down into one of the holes on the revolving disc plate and was amputated.
