Investigation Summary
At approximately 11:15 a.m. on June 2, 2015, Employee #1 was painting the exterior of a two story building. He placed a step ladder on a tiled roof top. The ladder was unsecured and Employee #1 slid off of roof approximately 20 feet landing on his head. Employee #1 was killed.
