Employee severely burned by fire and hot ash

At approximately 7:10 a.m. on March 13, 1992, Employee #1 and a coworker, of Crown Pacific, were on a 12 ft 6 in. platform cleaning a rotary valve that contained ashes and clinkers. The rotary valve is at the base of an ash hopper cone that separates hot ash from the gases that come from the boiler furnace. The boiler furnace supplies heat for the kiln to dry stacks of lumber. The ashes fall down the ash hopper cone through the rotary valve and into the ash box that is removed by a forklift and dumped. The coworker descended from the platform to move the forklift so ashes would not fall on it. Employee #1 opened the 16 in. by 21-1/2 in. door at the base of the ash hopper cone, oxygen hit the hot ash, and a ball of fire engulfed Employee #1, causing him to fall 12 ft 6 in. to the concrete while on fire. Hot ashes poured down on him. Three coworkers put out the fire. First aid was administered and Employee #1 was transported by ambulance to Prineville Hospital. He was later airlifted in critical condition to Emanual Hospital in Portland.
