Investigation Summary

At approximately 1:30 p.m. on March 30, 1991, Employee #1 was measuring an approximately 30 ft tall window inside a church. A church employee was going to tie off the extension ladder, but Employee #1 climbed the ladder before it was secured. When Employee #1 was at a height of approximately 20 ft, the ladder began to slide along the wall and he fell, striking his neck against a pew. Employee #1 was pronounced dead at the scene.
