Investigation Summary
At approximately 10:15 a.m. on May 10, 1990, Employee #1 was either knocked down or fell into the path of a 475 Barko hydraulic loader from the loader operator side, which is obstructed by the boom. A coworker on the landing attempted to get the loader operator's attention, but before he could, the loader had almost completely run over Employee #1, killing him. No county medical examiners were summoned to the scene.
