Investigation Nr: 997023
Event: 10/01/1989
EMPLOYEE DIES FROM ACUTE MYOCARDIAL INFRACTION

From 8:45 a.m. to 12:00 p.m. on October 1, 1989, Employee #1 and Employee #2 were working in an unventilated 32,900 cubic foot warehouse periodically operating a propane powered forklift truck. Employee #1 and Employee #2 then left and drove five miles to a parking lot and at 12:20 p.m. Employee # 1 passed out suffering a cardiopulmonary arrest and died. At 1:10 p.m. Employee #1 and Employee #2 had their arterial blood gas taken which showed 16.2 percent Coh6. At 3:30 p.m. Employee #2 was admitted to a hospital and treated with oxygen therapy. A second blood gas test of Employee #2 showed 14.4 percent Coh6. At 4:00 p.m. a detector tube probe of the forklift truck showed a CO emission concentration of three percent. The corner documented cause of death for Employee #1 was from acute myocardial infraction and not related to CO exposure.
