Investigation Summary

Investigation Nr: 14559884
Event: 09/12/1991
Employee killed when pulled into hay baler

Between 2:00 and 4:00 p.m. on September 12, 1991, Employee #1, a ranch hand, was alone cleaning hay from the plunger chute of a New Holland model 320 hay baler. Apparently Employee #1's dog, who always rode in the cab of the International model 1066 tractor with him, inadvertently brushed past the power take-off operating lever and activated the power from the tractor to the hay baler. Employee #1 had left the tractor running. He was caught by the tines that rake hay into the plunger chute and his upper torso was pulled into the plunger chute. Employee #1 sustained a fractured wrist and a severed right arm. In addition, one of the tines punctured the right side of the employee's chest, severing the axillary artery. Employee #1 bled to death.
