Employee killed when head strikes propane tank in fall

On May 29, 1991, Employee #1 and a coworker were attempting to repair a broken overhead door by tightening a tension spring located near the ceiling. They were standing 16 ft from the floor with one foot on a ladder and the other on the I-beam. Employee #1 and the coworker were facing each other; one man was pulling and the other was pushing on a rod that had been inserted to tighten the spring. The rod came loose and Employee #1 fell, hitting his head on a propane tank. He died.
