Investigation Summary
Employee #1 was working on a two-point suspended scaffold in excess of 100 ft above the ground painting the outside of a hotel. Employee #1 fell off the scaffold to the concrete area below. Employee #1 died of massive internal injuries. Fall protection was provided but not worn.
