EMPLOYEES DIES WHILE REMOVING SHINGLES ON A ROOF

Employee #1 fell and died while removing shingles on a 3-12 pitch roof. The distance from the ground to the eave was 18 feet and 7 inches. He apparently was not wearing any fall protection. He was standing at the gable end throwing old shingles from the roof to the truck. He slipped off the edge and hit his head on the rear edge of the truck. The distance from the edge to the ground was 22 feet. He died from a concussion.
