Investigation Summary
Employee #1 was making a routine check of a lift station (underground sewer station). The employee was descending a 28 ft fixed ladder when he fell and struck his head against concrete at the ladder base. Employee #1 suffered massive head injuries and died from the loss of blood. The lift station and ladder was manufactured by Smith Loveless, located in Lenexa, KS.
