Investigation Summary

Investigation Nr: 552273
Event: 05/03/1995
Employee dies in fall through skylight

Employee #1 was pulled from his regular welding duties to help electricians lay a 6-inch thick electrical cable in a cable tray. The cable tray was being installed for the construction of a new building and was located on top of the existing building's flat roof. The roof had several plexiglass skylights. As the workers moved the cable to the cable tray, a loop in the cable struck Employee #1 in the chest and knocked him backward. He fell through an unguarded plexiglass skylight 20 feet to the inner concrete floor. He died from his injuries.
