Investigation Summary

Investigation Nr: 568725
Event: 07/26/1993
Employee dies from heat stress

On July 26, 1993, Employee #1 was performing his normal job operation on the 3:00 p.m. to 11:00 p.m. work shift of operating the #1 seed cleaner machine. Shortly before 11:00 p.m., the seed cleaner machine broke down and maintenance personnel began repair work. Employee #1 was sitting on a trash can type container while the repair work was being performed. When maintenance returned from obtaining larger screws for placement in the attachment for the machine, they found Employee #1 lying on the floor between the #1 and #2 seed machine. They tried to wake him and continuously fanned him while help was summoned. The foreman on duty called Pafford Ambulance Service at 11:00 p.m. The unit arrived on site at 11:22 p.m. and began treating Employee #1 for heat exhaustion. The ambulance departed the site at 11:27 p.m. and arrived at 11:30 p.m. at the Helena Regional Medical Center. Employee #1 was treated in the emergency room. Employee #1 was pronounced dead at midnight July 27, 1993. Autopsy report listed cause of death as environmental hypothermia.
