Investigation Summary
At 2:20 p.m. on July 2, 2015, a nurse was performing a face check of patients (routine physical verification of patients whereabouts). The nurse had refused to allow a patient to attend a pet therapy session, as the patient had refused to get up. The nurse was in the chart room within the PIC (Patient Information Center), when the patient jumped over the counter, went into the chart room and commenced hitting the nurse in the head, resulting in injuries to her head and left eye.
