Investigation Summary
At approximately 10:15 a.m. on March 11, 1995, Employee #1 fell through a skylight that was not guarded and/or did not have a cover strong enough to hold him. Shortly before the accident, fellow employees saw the employee walking across the roof holding a cup of coffee. After they turned their backs to him, they heard a crash. It is believed the employee either sat down on the skylight or stepped across it as a shortcut. He fell 23 feet 3 inches to the concrete warehouse floor. He died at the scene from multiple head injuries. The skylight was 4 feet 6 inches wide by 8 feet 6 inches long and overlaid with an old, weathered plastic cover. There were no witnesses to the accident.
