Employee dies after fall into monorail conveyor

At approximately 12:00 noon on October 7, 1994, Employee #1 was checking on a jammed indication conveyor which feeds a monorail bucket conveyor system that handles bulk mail sacks. As he descended the stairs to the maintenance catwalk, stepped around the ladder, and proceeded toward the conveyor which was running perpendicular to him, the employee tripped. He either tripped over used belting lying on the floor or tripped due to an uneven 1 in. step down on the floor, and fell into the moving monorail conveyor at the point where it was not protected by a guardrail system. The monorail pulled the employee into a pinch point where guides were attached to prevent the side movement of buckets during loading. Employee #1 was pinned between a bucket and a guide. He died immediately of crushed ribs, a crushed liver, and other internal injuries.
