Investigation Summary
At approximately 3:00 p.m. on August 18, 1994, Employee #1 and three coworkers were attempting to lower a 10 inch diameter, 3 section drain pipe into a trench that measured 60 feet long by 4 1/2 feet wide by 12 feet deep. When the pipe was lowered into the trench by the excavator using a chain sling, the pipe came apart. A coworker went into the trench to connect the section and yelled for assistance. When another coworker went in to help, the wall collapsed around him. Employee #1 jumped in the trench to dig him out. The wall collapsed further and completely buried Employee #1. Rescue personnel dug one worker out safely, but could not save Employee #1, who died. Another coworker escaped the cave-in by running in the opposite direction. No ladders, sloping, shoring, or trench box were in use, and the spoil bank was piled up on the edge of the trench.
