Employee dies in fall from moving scaffold

On June 16, 1994, Employees #1 and #2 were working from a suspended scaffold inside Hold 4 of the SKS Star (a Liberian-registered ship). They were using chemical and cleaning apparatus to clean the inside of Hold 4 in preparation for receiving a cargo of grain. The ship's crane (15 ton), rigging, and gangway were in use. As Employees #1 and #2 were being lifted from the hold, it is believed that the lifting cables came into contact with a coaming edge around the hold. One of the cables snapped, causing the scaffold to fall. Employees #1 and #2 fell approximately 70 feet to their deaths. They were not wearing safety belts or lanyards attached to independent lifelines.
