Employee killed when caught between roller and belt

Employee #1 climbed to the catwalk of a conveyor belt system to oversee material that was flowing irregularly on the belt. The employee was found caught between the roller and the belt after the conveyor belt system automatically stopped. He was killed. The bottom portion of the belt system was not guarded.
