Investigation Summary

Employee #1 (the employer) was working with three employees on a 14 foot scaffold pick board installing sheetrock. No personal fall protection or guardrails were in use. While Employee #1 and a coworker were trying to put a piece of sheetrock in place overhead, Employee #1 took a step forward and to the right, to adjust his position, and fell off the scaffold board. He struck the left side of his head and left shoulder on the concrete floor, 14 feet below. He suffered massive head damage and died.
