Investigation Summary
At approximately 10:30 a.m. on April 19, 1991, Employee #1, a journeyman ginner, was preparing to repair a 12-in.-diameter air duct approximately 22 to 23 ft above the floor. Employee #1 instructed the operator of a the Nissan Model 40, Type 6, lift truck to hoist him and his helper up on a pallet to repair the duct. The helper got off the lift truck onto a horizontal channel iron at a height of approximately 11 to 12 ft. Employee #1 continued up to approximately 18 feet. When he placed his weight on the pallet, it dislodged, causing him to fall to the ground. Employee #1 struck his head against the concrete floor and was killed.
