Investigation Summary
At approximately 2:50 p.m. on July 6, 2015, Employee #1 was rigging a catwalk/pipe handler to be lifted off a flatbed trailer. The chains and hooks were wrapped around the channel iron of the catwalk/pipe handler. As the equipment was being lifted, the rigging failed causing the catwalk to fall and turn on its side striking Employee #1 in his back and killing him.
