Investigation Summary
At approximately 1:45 a.m. on June 25, 2015, Employee #1, a foreman, was retrieving and returning materials to his work truck that was parked on the shoulder of a highway. A privately owned tractor trailer came into the work zone, jack knifed and struck Employee #1. Employee #1 was killed.
