Employee dies when struck by falling bundle of scrap paper

The shop foreman was inventorying the stock in the scrap paper storage area where the forklift operator was using a Caterpillar, model V50, forklift to load brown scrap paper bundles onto a truck going to a recycle yard. The brown scrap paper storage row had a forward lean of approximately 10 degrees on stacks of six bundles. The forklift operator had loaded about half the truck and was loading a three-bundle stack onto the truck when Employee #1 came into the area to talk with the shop foreman. The stack of six bundles behind the one being loaded fell forward onto Employee #1. Employee #1 was struck and fatally crushed by one or two bundles of brown cardboard scrap paper, each weighing approximately 1,140 lb.
