Investigation Summary
Employee #1, a foreman, was 92 ft high in a one-man basket that was attached to the jib extension of a boom. After the operator told him that the basket was not to be used from the jib extension, the employee told the operator to let him down. As the operator lowered the basket, the arm on the basket caught on a beam. Before the employee could tell the operator to stop, the sheave, with the basket still attached, was pulled off the jib extension and fell 90 ft to the floor. Employee #1 was killed.
