Employee falls from ladder and is killed

At approximately 4:30 p.m. on June 28, 2015, Employee #1 was on a ladder that was approximately 10 feet trying to add a threaded rod to support a panel box on the ceiling. The only tools that were in his possession were his tool belt and he had no tools in his hands. Employee #1 fell, suffered head trauma, and died.
