Employee killed when forklift overturns

Employee #1 was using a forklift to carry one end of a 7,000 lb pipe that was 65 ft 10 in. long and 24 in. in diameter. A coworker on another forklift was carrying the other end of the pipe. Employee #1 was traveling backward and turned too sharply to avoid a pole. His forklift tipped over, pinning his head beneath the forklift's overhead protection rack. Employee #1 was killed.
