EMPLOYEE #1 WAS ASSISTING THE EQUIPMENT OPERATOR IN STARTING THE CATERPILLAR PIPELAYER WHEN IT STARTED REVERSING. THE PIPELAYER MOVED BACKWARD ON ITS TRACK, CRUSHING EMPLOYEE #1 BETWEEN ITS 7-INCH OPENING AND KILLING HIM. EMPLOYEE #1 WAS NOT PROPERLY TRAINED IN THE RECOGNITION AND AVOIDANCE OF UNSAFE CONDITIONS INVOLVING EQUIPMENT STARTUP.
